Insurance:

Be sure to bring your Insurance Identification Card and any other necessary information to make this process quick and relatively simple.

Although, our office will file your insurance claim, you are required to submit the deductible or co-pay amount at the time of your appointment. Proof of deductible is required.

It is a patient's responsibility to provide a referral form if necessary for your insurance company.

Medicare:

Our office accepts medicare assignments. You will be responsible for remaining charges medicare does not pay. Our office will file the medicare claim form for you.These forms must be obtained from your insurance company. In order for our office to file supplemental insurance, you must supply our office with a current insurance card.
