Many
patients are sent to us by referral of their family physician. Dr
Schoenfeld will notify your Doctor of any diagnosis or treatment.
You are billed separately for Dr. Schoenfeld's service as a specialist.
Payment is due in full at the time service is rendered. If you have
insurance coverage, our office will be happy to file the claim form.
You are responsible for the deductible or co-pay amount at the time
of service. If for some reason your insurance company does not pay
the remaining balance, you must do so immediately upon notification.
